Healthy and Well Kids in Iowa is a state-wide program for uninsured children that provides no-cost or low-cost health care coverage to children in working families. Familes pay no more than $20 per month and children get an insurance card and all of their health care services through a health plan that has agreed to participate in the program. To qualify for hawk-i, a child must:
- Be under 19 years old
- Be uninsured
- Be ineligible for Medicaid
- Be a citizen or a lawfully admitted resident alien
- Meet the income guidelines
Current income guidelines for hawk-i are set at 300% of poverty. For example, a family of four could make up to $66,150 per year and still qualify for hawk-i health coverage. Families must provide proof of citizenship and identity. Each application requires a one month waiting period.
What services are covered through hawk-i?
Many people might be concerned about the type of services covered through the hawk-i health coverage plan. The hawk-i health plan offers an incredible amount of coverage for children. The following services are covered under the hawk-i health plan:
- Doctor visits
- Hospital care
- Prescriptions
- Well-child visits
- Vaccines/shots
- Emergencies
- Surgeries
- Dental care
- Vision exams
- Hearing services
- Mental health and substance abuse care
- Chiropractic care
- Speech therapy
Dental Coverage
Hawk-i now offers dental only coverage for children who may already have health insurance but may not have dental coverage. Similar to hawk-i health insurance, the dental only coverage is offered to families at or below 300% of the federal poverty level, meaning a family of four could make up to $66,150 and still qualify for dental only coverage. Monthly fees may be charged for income levels over %150 of the federal poverty level, but will not exceed $30 per month. The following services are covered under the hawk-i dental only coverage plan:
- Cleanings and check-ups
- Fluoride treatments
- Sealants
- Fillings and crowns
- Medically necessary orthodontia (Prior approval is required)
